Central Kitchen Field Trip Form

This Field Trip Form  needs to be filled out and sent into the Food Service Department. Email is best since this is a fill able form  on the computer but you may also fax or hand deliver it to any Kitchen Manager at your site or the LBE Central Kitchen.

Please note

• The food items are subject to change

• We need the information as early as possible for (ordering purposes etc.)

• The students listed on the field trip form need to come and pick up their own lunches in the morning from the cafeteria.
